Single Point Referral Process
The referral process to request support for children and young people in mainstream settings with unmet Social, Emotional & Mental Health (SEMH) or other medical needs is via Single Point Referral.
Head teachers attend and chair a fortnightly meeting to consider referrals. The current form can be found below.
It brings together the processes for referrals to education through a Single Point of Referral following a review of actions and outcomes for the child via a My Support Plan.

This guidance has been developed primarily to support mainstream schools (maintained schools, academies and free schools) in Kirklees but will also be useful to other professionals across education, health and social care who work with schools in supporting children and young people with SEN.
